	If you're interested in getting a puppy from us, please fill out this form. Most of the questions are optional. However, the more information you provide, the better we can understand your needs. Of course, Ruskai will keep your answers confidential, and will use the information only as an aid to help us select the best possible homes for our puppies. Please note that Ruskai cannot guarantee a puppy to everyone who fills out this questionnaire: it is simply a form that helps us evaluate potential adoptive homes. Being on our waiting list or completing and sending a form does not guarantee that you will be able to purchase a puppy from us.
